At Experience Professional Care Solutions, we’re committed to ensuring our clients receive personalized, seamless care—even across state lines. Recently, our team leader and nurse, Jen Krum, had the pleasure of traveling to the beautiful Lake Oswego, Oregon, to assist with the safe transition of our dear client, Kay.
During the visit, they took some time to explore the local area and enjoyed a special treat at the iconic Salt & Straw ice cream shop. It was a heartwarming moment as Kay’s grandson, Nathan, joined them for an afternoon filled with laughter and, of course, ice cream!
The unique flavors were a highlight, including a wild blue cheese and pear and the intriguing combination of strawberry, balsamic, and black pepper.
These simple moments remind us of the joy and connection that personalized care brings—not just through medical support but also through life’s sweet experiences.

When a Loved One Must Move Far From Home
Moving an aging loved one to another city or across state lines can bring a mix of emotions. Your family may feel hopeful about the next chapter. At the same time, you may feel overwhelmed by the planning, the travel, and the many details that need attention.
You may worry about your loved one’s comfort during a long drive or flight. You may wonder how they will manage medications, meals, bathroom breaks, mobility needs, fatigue, or confusion in an unfamiliar setting. These concerns are especially common when a parent has limited mobility, memory changes, chronic health conditions, or anxiety about leaving a familiar home.
Families also often worry about safety during each part of the transition. Can Mom get in and out of the vehicle safely? Will Dad have enough support in airports, hotels, rest areas, or unfamiliar homes? Who will notice if they become tired, uncomfortable, disoriented, or unwell along the way?
A long-distance move can also create emotional concerns. Your loved one may be leaving friends, neighbors, routines, healthcare providers, and a home filled with memories. Even when the move is necessary or positive, it can bring grief, uncertainty, and fear of losing independence.
Thoughtful Support During the Transition
A successful transition begins with planning around the individual, not simply the destination. Families may need to consider the travel schedule, mobility needs, medication timing, meal and hydration needs, comfort items, medical appointments, and the level of assistance required throughout the trip.
It can help to plan for regular rest breaks and avoid rushing. Familiar snacks, water, comfortable clothing, medications, mobility devices, important documents, and a favorite blanket or personal item can help your loved one feel more settled. Whenever possible, include them in conversations and decisions about the move.
Professional support can bring added reassurance during this process. A trained caregiver or nurse can help monitor comfort, assist with mobility, support personal care needs, encourage meals and hydration, and offer calm companionship during travel. They can also help families communicate clearly and respond thoughtfully when plans need to change.
Experience Professional Care Solutions understands that transitions are about far more than getting from one place to another. They involve protecting a loved one’s safety, dignity, routines, and emotional well-being throughout a major life change.
Care That Travels With Your Family
Our team is proud to help clients and families navigate transitions with compassion, careful attention, and personalized support. Whether a loved one is moving closer to family, relocating for a new care arrangement, recovering after a hospital stay, or transitioning between homes, we focus on making the experience feel more manageable.
That may include helping a client feel comfortable during travel, supporting their daily routine, providing companionship, and helping them settle into a new environment. Just as importantly, it can give family members the confidence that they do not have to manage every detail alone.
At Experience Professional Care Solutions, we are proud to make transitions smooth and enjoyable for our clients, no matter where they are.
